Gather evidence

Custodian reporting and your own records work together. Form 1099-R concerns distributions; Form 5498 concerns IRA contribution information. Reconcile them with statements and your tax records.

  • Historical basis records and relevant Forms 8606.
  • Year-end statements for relevant accounts.
  • Contribution confirmations with designated tax year.
  • Conversion and rollover transaction confirmations.
  • Plan acceptance documents for a proposed reverse rollover.

A readiness score has limits

A completed checklist indicates organization. It does not confirm the accuracy, sufficiency or legal treatment of the documents.
